Liability Waiver and Release of Liability

I accept the position of Dancer/Auditioner (circle one) on the Colorado King's Lady Dancer's team for the 2012-2013 season. Activities include rehearsals and all events and activities organized by the Colorado King's and the Colorado King's Lady Dancers.  I certify that I am in god mental and physical condition.  I understand the inherent risks associated with performing dance activities.

I agree not to sue and forever release, waive, and discharge Colorado King's, American Basketball Association, Colorado King's Lady Dancers, Visions Performing Arts Company and their respective employees, governors, affiliates, agents, partners, owners, members, parents, subsidiaries, representatives, officers, attorneys, and players (hereinafter referred to as "Releases") from any and all liability to me, my child(ren), my personal representatives, assigns, heirs, dependents, pets, spouse and relatives for any and all claims, causes of action, losses, judgements, liens, costs, demands, that are caused by or arise from any injury, including death to me  or my property regardless of the cause(s) of such injury.  I assume all risks associated with my participation in and observation of the Activities.

I further grant Releases the perpetual worldwide and royalty-free rights to use my voice, photograph, and likeness in any media related to my performance in or observation of the Competition including, without limitation, a videotape recording of such Activities without compensation to me, or my personal representatives, assigns, heirs, children, dependents, spouse, or relatives.

I hereby acknowledge that I have carefully read this Liability Wavier and Release of Liability, that I fully understand its content, that I am signing this Liability Waiver and Release of Liability voluntarily
and intend for it be legally binding.
